Overview – Illinois HEN • Partnership – IHA & HRET • 2000 hospitals in 33 states • Goal by 2013 (2 year project) • Reduce harm by 40% • Reduce readmissions by 20% • Equip hospitals with leadership, change management, facilitation, quality improvement & teamwork skills • Focus on improving the safety culture

  3. Overview – Illinois HEN • Partnership – IHA & HRET • Areas of focus: • CLABSI • CAUTI • Surgical Infection & Complication • VTE • Adverse Drug Events • Falls • Birth related injuries • Pressure Ulcers • VAP

Overview – Illinois HEN • Partnership – IHA & HRET • Partnership for Patients funded resources • Help to identify HAC’s in need of intensive & moderate support • 3 levels of improvement activity • Stand-alone webinars (60-90 mins) • 2 fellowships (8months) • Collaboratives (12-15 months) • Monthly coaching calls • Monthly affinity group calls • Topic specific interventions of proven impact

Overview – Illinois HEN • Partnership – IHA & HRET • Partnership for Patients funded resources • Topic specific guides, toolkits, guidelines & checklists • Ongoing access to subject matter experts in all 10 areas • Targeted interventions for specific hospital types (children’s, LTACH, psychiatric) • Intensive support for hospitals or regions with significant challenges

Overview – Illinois HEN • Partnership – IHA & HRET • Partnership for Patients funded resources • Data collection and Progress monitoring & support • Selection of measures that minimize data collection burden • Support of multiple data entry modalities • Performance Progress reports • Monthly to hospitals units • Quarterly to hospital leadership • Dedicated program website where all calls and webinars will be archived along with written material and important links

Overview – Illinois HEN • Partnership – IHA & HRET • Hospital Requirements • Complete Needs Assessment by February 24, 2012 • Select 2-3 HAC’s for intensive focus & 3-4 areas for moderate focus – early March • Complete Hospital Improvement Plan – Q2 2012 • Identify hospital improvement effort lead – Q2 2012 • Identify Project Team Lead and Team Members – Q2 2012 • Attend Illinois Kick-Off Meeting – Q2 2012 specific date TBD • Submit monthly data (1 process, 1 outcome per HAC)Q2 2012 • Select fellowship participants – Q2 2012
