  2. Définition • Outil créé par HAS • Maladies chroniques : enjeu de Santé Publique majeur • Définit un plan d’action pour la PEC prospective • OBJECTIFS : • Implication du patient • Coordination Patient / Professionnels de Santé • Harmonisation des pratiques et personnalisation du parcours

PEC globale : 2 axes • Personnalisation de la prise en charge (tableau 1) • situation médicale et sociale • point de vue/attentes • patient • médecin • les freins • définition des priorités de la PEC • Planification du suivi ( tableau 2) • patient : se repérer • professionnels : information commune partagée • réévaluer les propositions et réajuster

  4. Objectifs médicaux • Identifier les FdR et ttt • Tabac • Exposition professionnelle • Initier un traitement • Médicamenteux • Réhabilitation respiratoire • Intervenants: MT, pneumologue, pharmacien, professionnels de la RR, service sociaux, association de patients • Planifier le suivi • ETP  Diminuer les exacerbations et les hospitalisations

  5. Mr D. 59 ans • Cs après hospitalisation pour une exacerbation de BPCO en janvier 2012. • BPCO stade III diagnostiquée il y a 4 ans • Tabagisme actif : 50 PA • BMI : 35 • Dyspnée à 1 étage • mauvaise observance
