  1. Instructions Do not use this informational slide as part of the disclosure template. What do I need to do to comply with the Disclosure Policy? Every planner, course director, CME associate, speaker, moderator, panelist or anyone else involved in the CME activity that may have influence over the content of the CME activity, must disclose financial relationships with commercial interestsBEFORE the CME activity begins. This information must be disclosed to the participants before content is delivered. The easiest way to do this is to put the disclosure information on a PowerPoint presentation that loops (select Slide Show, select Set Up Show, select Loop Continuously until ‘Esc’, OK) through the slides while the participants are coming in and at the beginning of the opening session. The next slide (Slide 2), is the UAMS Disclosure Policy that needs to be displayed in a PowerPoint presentation or placed in the handouts, or both, PRIOR to the conference presentations begin.

UAMS Disclosure Policy It is the policy of the University of Arkansas for Medical Sciences (UAMS) College of Medicine to ensure balance, independence, objectivity, and scientific rigor in all sponsored or jointly sponsored educational activities. All individuals who are in a position to control the content of the educational activity (course/activity directors, planning committee members, staff, teachers, or authors of CME) must disclose all relevant financial relationships they have with any commercial interest(s) as well as the nature of the relationship. Financial relationships of the individual’s spouse or partner must also be disclosed, if the nature of the relationship could influence the objectivity of the individual in a position to control the content of the CME. The ACCME describes relevant financial relationships as those in any amount occurring within the past 12 months that create a conflict of interest. Individuals who refuse to disclose will be disqualified from participation in the development, management, presentation, or evaluation of the CME activity.

Instructions Do not use this informational slide as part of the disclosure template The next slide (slide 4) is a template to use for the planners/speakers/moderators/panelists who have NO financial relationships with commercial interests to disclose. Slide 5 is a template to be used if you have planners/speakers/moderators/panelists that DO have a financial relationship to disclose. It is best to use a separate slide for each person who has a financial relationship to disclose. Sometimes, you may need to use both of these slides if you have individuals who have financial relationships to disclose and those who do not.
