Positive transitions Young people leave school to experience new activities and to be able to do things which have meaning to them Parents want son/daughter to be happy, and to have security for the future Students have the option of an individual budget (funding streams include health, social care, ILF, learning (LSC) Families know what is happening before the end of the summer term term

Key Feature: Curriculum for citizenship Voice, choice and experience • What makes you happy? • How do you communicate? • How do you make decisions? • Who are the important people in your life? • What would you like to try? • How do you like to be supported? What do you need to make things work for you?

  6. Key future: Co-ordinated support • Bringing agencies together to work on agreed plan for transitions each year • Clarity around roles and responsibilities • Going the extra mile • Support planning involves all agencies • Talk, talk, talk

  9. Outcomes… Young people and families opting for a mix of traditional services (college, day centres in voluntary sector + local authority), and more creative options (music workshops, allotment, Gym membership, Personal Assistants, private Tuition (e.g. IT), micro enterprise
