WHY AGAIN? • 10/Oct Death - overtaking a truck • 20/Oct Death - motorbike lost control • 21/Oct LTI - rollover on graded (joy-riding?) • 30/Oct LTI - rollover (permanent disability?) • 1/Nov LTI - hand caught under heavy spool • 4/Nov LTI - hand caught in beam-pump • 6/Nov LTI - rigger falls off truck • 9/Nov LTI - rollover on blacktop 30 days: 8 families suffering

Discuss in your team (10/October fatality):If you were the land-cruiser driver(he was overtaking!):- How could you have prevented this accident?- Why did he not do these things?- What protected the driver? Defensive Driving on the long blacktop road: be prepared for the unexpected!!!Apply the 4 "R"s:1 READ the road and traffic as far ahead as possible.2 Drive at the RIGHT side of your lane.3 REDUCE your speed when you see oncoming cars thatcould pull onto yourside of the road.4 RIDE-OFF the road onto the shoulder or into the desert if needed. You MUST avoid a head-on collision.As a passenger:- Do point out the HAZARD to the driver. 4 eyes are betterthan 2. He might not have noticed it.

  9. 9/Nov: Hammed Al-Abri, broken shoulder & fingers, cuts Trip from Muscat to Yibal. Veered off to the right.Overcorrected rolled 3x.Tired?Concentrated?Avoid animal?Broke night driving rules! Blacktop near Fahud: NIGHT Roll-over !!

  10. 16/Nov: Graded road Bahja NEW!! Roll-over !! Look at the classic S-pattern.Driver: 19 yrs old. Asleep?Badly hurt his elbow: LTI. 3 passengers: minor injuries.All had their seatbelt on: good!

  11. Discuss in your team: ROLL-OVERS- Why do roll-overs happen?- Have you had one, or almost? Why did that happen?- Are roll-overs caused by: - The car? - The road? - The driver?- How do you prevent roll-overs: as a driver?- How do you prevent roll-overs: as a passenger?- How do you prevent roll-overs: as a supervisor?

  12. Why do roll-overs happen?: • Driver speeding • Driver not concentrating • Driver tired • Driver poor vision: night or dust • Driver wrong reaction: S-pattern and braking • Tyre blow-out (not often)

How can you prevent roll-overs?: • Speed?Slow down! • Concentration?Concentrate! • Tired?STOP, rest, have a nap! • Night?Do NOT drive at night! • Dust?Do NOT go into a dust-cloud! • Wrong reaction?Train yourself mentally: NOT to brake! NOT to over-steer! • Tyre blowouts?Check pressureregularly! Check condition regularly! Replace poor tires!

How to protect yourself against roll-over? • Use a 2-WD instead of a 4-WD.4-WD have a high center of gravity.This makes it more sensitive to roll-over.If you drive a 4-WD: adjust your driving. • Make sure a 4-WD has a ROLL-OVER BAR !!!!! • Wear your seat belts !!!!! Your BELT will Save your LIFE

  15. 1/Nov: Suleiman al Jabri, 23 yrs, broke 2 fingers.Unscrewing the valve assembly.It suddenly released.Suleiman tried to hold it, but he fell over with the pipe trapping his fingers onto the drill floor.

  16. 4/Nov: Hameed al Walibroke his finger +bad cuts in hand.Trying to open a rusty jammed lock mechanism of a beam-pump.It suddenly released and smashed his hand.

  17. 6/Nov: Khalfan al Shuaily Head injury Khalfan Loading a skid onto a trailer. Khalfan was still unhooking the slings when the driver drove-off. Khalfan fell-off the truck. The load fell over him. He is lucky to be alive!

  18. What went wrong in these last 3 incidents? • Had hazards been identified before starting the job ?? What about the controls ?? • Was there ACTIVE supervision ?? Who was in charge ?? • Did anyone check the procedures ?? • Was there a tool-box talk/discussion ?? • Were the guys experienced ?? If not: did someone coach them ?? • What about communication between team members ?? Do you have answers for these in all your jobs?? You better have!!

  19. Is YOUR Safety important ??to you ? YESto your family ? YESto your company ? YESto PDO ? YESThen follow these very basic steps:- Toolbox meeting before every job: always- Identify hazards, look for them, be aware of them: always - Protect yourself from these hazards, control them: always- If you can not control them, if it is not safe: STOP alwaysDiscuss in your team: your jobs, your hazards your controls.
