  1. Hoops For Heart Online Registration DO NOT SIGN-UP Slip We will be having a sign-up for the online fundraising portion of Hoops for Heart during PE class during the week of March 21-25. The Hoops for Heart Event will run from April 15th through April 28th. The online site allows participants to track their individual & team progress while having access to the AHA’s educational and fundraising resources. Students will need a valid e-mail address to sign up. By signing this form, I do not give my child permission to register for the Hoops for Heart online fundraising program while at school. This form is due back by March 21st.
