A reporting tool created by the Nevada Department of Agriculture to track schools' progress in meeting local wellness policy goals. Includes website addresses for school policies, information on events and activities, and ways for individuals to get involved.

Why do we have this tool? • 2010 HHFKA • 2014 Local School Wellness Policy Interim Rule put forth reporting requirements • Annual- • Website address for each school policy and/or how the public can receive/access a copy of the local wellness policy • Description of each schools progress in meeting the local wellness policy goals • A summary of each schools events or activities related to local school wellness policy implementation • The name, position title and contact information of the school wellness policy coordinator • Information on how individuals and the public can get involved with the school wellness policy team • Triennial- • Indication of the extent to which schools under the jurisdiction of the LEA are in compliance with the LWP • Indication of the extent to which the local wellness policy compares to the model school wellness policies • Description of the progress made in attaining the goals of the local school wellness policy

  3. How Does the Tool Work? • NDA created an online reporting system • All SFA’s were asked to designate a Point of Contact (POC) • The POC will receive an email with their login information • They will need to input the district level data • Once they have submitted this, the school/site level data can be collected http://wellness.agri.nv.gov

  11. Due Dates • All schools should complete their reporting by JUNE 1st, 2015 • Sponsors should begin NOW to ensure timely completion
