    12. Our Evolution June 2001 Federal CHIPP project with 19 sites known as Eastern Ontario Telehealth Network (EOTN) March 2003 Recognized as provincial program New Sites - South East and Champlain New Name “CareConnect” Today 27 partners on more than 50 sites
