EXTENDED APPOINTMENTS FOR SCHOOL AND SPORTS PHYSICALS WILL BEGIN 06/02/11. THE COMPLETE PHYSICAL WILL BE DONE IN ONE APPOINTMENT IN THE PEDIATRIC CLINIC. THE APPOINTMENT WILL TAKE APPROXIMATELY TO 1 TO 1 ½ HOURS.

  1. EXTENDED APPOINTMENTS FOR SCHOOL AND SPORTS PHYSICALS WILL BEGIN 06/02/11. THE COMPLETE PHYSICAL WILL BE DONE IN ONE APPOINTMENT IN THE PEDIATRIC CLINIC. THEAPPOINTMENT WILL TAKE APPROXIMATELY TO 1 TO 1 ½ HOURS. PLEASE HAVE THE PARENT PORTION OF THE PHYSICAL FORM COMPLETEDPRIOR TO YOUR ARRIVAL. THE VIRGINIA SCHOOL ENTRANCE FORM FOR KINDERGARTEN OR ELEMENTARY SCHOOL MAY BE DOWNLOADED FROM THE WEB SITE: http://www.vdh.virginia.gov/Epidemiology/Immunization/documents/SchoolForm.pdf THERE IS NO PHYSICAL REQUIREMENT FOR ENTRANCE INTO PUBLIC MIDDLE OR HIGH SCHOOL IN VIRGINIA. A SPORTS PHYSICAL FORM MAY BE DOWNLOADED FROM YOUR SCHOOL OR COUNTY WEB SITE. FOR TRICARE PRIME CHILDREN ASSIGNED TO RADER PEDIATRIC CLINIC • THE CHILD MUST BE ACCOMPANIED BY A PARENT. • PLEASE BRING THE FOLLOWINGTO THE PHYSICAL APPOINTMENT: • THE CHILD • MILITARY ID • SHOT RECORD • MEDICAL RECORD • GLASSES OR • CONTACT LENSES • PHYSICAL FORMS WITH THE PARENT’S SECTION COMPLETED CALL 703-696-7951 TO BOOK A PHYSICAL APPOINTMENT.
