Recommendations • Timely, good quality assessments at the start of an order • Timely, good quality assessments of vulnerability & Risk of Harm as appropriate • Intervention Plans to clearly link to assessments and to be reviewed regularly • Continuity between different plans (Intervention, Vulnerability, Risk of Harm) • Regular quality assurance by managers with actions followed up by staff
