Enhancing Quality and Access to Care: Empowering Service Users and Carers
This initiative focuses on improving the timeliness and ease of access to services for users and their carers. By adopting the philosophy of “no decision about me without me,” we prioritize listening to service users and incorporating their perspectives in service design, delivery, and evaluation. We ensure each service user receives thorough assessments and tailored care plans, with timely, accurate recording of interventions. Collaboration across service boundaries is essential to provide seamless care, while safeguarding the well-being of all stakeholders involved in the care process.
