PATH Data Collection Choosing a PATH Project in HMIS • PATH providers must obtain at time of initial contact a response to the question, “Where did you stay last night?” • Individuals living in places not meant for human habitation (streets, encampments, abandoned buildings, cars, public places, etc.) are to be entered into the Street Outreach Project. • Individuals living in places meant for human habitation (shelter, couch surfing, at-risk of homelessness, etc.) are to be entered into Supportive Services Only Project. • If an individual does not know where they stayed last night or chooses not to provide this information, the PATH provider is to enter the individual into the Supportive Services Only Project.

FAQ Engagement • Engagement date • The point at which an interactive client relationship results in a deliberate client assessment (sign onto case plan) • Data Quality starts on this date(for HUD DQ report) • Date of PATH Status Determination • Is the date of federal PATH Program enrollment if Enrolled in PATH is Yes • Can be the same as Engagement Date
FAQ Auto Exit Automatic Exit • PATH clients who have not been contacted for 90 days will be automatically exited from the HMIS system. DBHDD intends for PATH Program providers to work diligently so that no client exits the project with an automatic exit, an indication of disconnection from service. While auto-exits are not entirely avoidable due to the transient nature of some PATH clients, DBHDD recommends sparing use of the automatic exit.
FAQ Re-engagement Re-engagement • To be considered re-engaged/receive the service of Re-engagement, PATH clients must not have received a contact, service, or referral from the PATH team for a minimum of 15 days. Re-engagement may not occur once an individual has been exited from the system.
Data Quality- Red Flags • Engagement date = project start date = date path status determined • Community Mental Health Service • # of clients contacted = # of clients enrolled • No contact service within the report year
APR changes for 2019 • Removed Q 26 and Q 27 • Exit destination will be included in report • We are expecting that it will be a required csv upload
