IT Matters • Form is to be filled in electronically and then: • Printed off and 3 hard copies provided • One soft copy put on a CD • One hard copy of accounts for past 3 years • The form is locked so only the check boxes and areas with grey boxes can have text entered • Version of PQQ will be Windows 97 – 2003

How responses will be assessed and scored • 5 -where the submission is exceptional and offers above the requirement  • 4 - where there is very strong evidence/highest level of confidence that the organisation is able to meet the requirement • 3 - where there is strong evidence that the organisation is able to meet the requirement • 2 - where there is some evidence that the organisation is able to meet the requirement • 1 - where there is little evidence that the organisation is able to meet the requirement • 0 - where there is no apparent evidence that the organisation is able to meet the requirement • Yes/No answers Yes 4 No 1

  8. References • Provide two referees, who will enable VSO to independently verify your experience and details. • A referee cannot be an employee or board member of the VTI/Organisation. It could be another funder, NGO, local trade association or business. • Sort out who would be your referees now .

Make sure that they send it back otherwise you will be disqualified
