  4. Licensed Health Professional Credentials • Licensed/registered therapist, counselor, or practitioner • Copy of current license or registration as behavioral, medical, or social services practitioner • If pets or horses are used in mental health treatment, proof of training is required

  5. Art Therapist Credentials • Current certification or registration from Art Therapy Certification Board (ATCB) • Evidence showing completion of Master’s in Art Therapy degree • Seeking registration as an art therapist/under supervision of an ATR

  6. Equine-Assisted Psychotherapist Credentials • Licensed/registered psychologist, behavioral therapist, mental health disorder specialist, social worker, or physical/ occupational therapist • Qualified to provide therapy using animals • Documentation of training, licenses & certifications • Two years of experience providing Equine-Assisted Therapy • Current liability insurance certificate with at least $1 million CSL • Criminal background checks, no older than one year, for staff that may come in contact with members • Current business license & tax ID or Social Security number

  7. Equine-Assisted Therapist Credentials • Proof of training as Equine Therapist • Two years’ experience providing Equine-Assisted Therapy • Employment at an Equine Therapy center that meets credentialing requirements

Equine Center Credentials • Current membership in at least one professional association such as: • Equine Assisted Growth & Learning Association (EAGALA) • Professional Association of Therapeutic Horsemanship (PATH) • Human Equine Alliance for Learning (HEAL) • National Association of Certified Professionals in Equine Therapy (NACPET) • Those engaging in Animal-Assisted Therapy must be adequately trained

Equine Center Credentials Cont’d. • Horses used must be certified by one or more of the following: • Certified Therapy Horse Association (CTHA) • International Association of Equine Assisted Psychotherapy (EAGALA) • Therapy horses must be: • Examined by a licensed veterinarian within previous 12 months • Certified as able to participate in the program • Receive annual exams & certification • Safety equipment for all riders including riding helmets, safety belts & appropriate saddles/bridles

Equine Center Credentials Cont’d. • Equipped to protect all therapy horses, including adequate space for: • Each horse to be housed & exercised • Disposal of waste • Appropriate equipment & supplies to feed & water each horse • Equipment to properly groom each horse • Satisfy all national, state & local standards for proper, humane treatment of animals • Current liability insurance certificate with at least $1 million CSL

Equine Center Credentials Cont’d. • Criminal background checks, no older than one year, for staff that may come in contact with members • Staff or volunteers with convictions of felony, abuse, sexual misconduct, or abuse of animals must be excluded • Horses must have at least two years of experience in handicapped/therapeutic operations or as a school horse used in a daily lesson program • Current business license & tax ID or Social Security number

PATH Premium Accreditation • Evidence of current Premium Accreditation • Current liability insurance certificate with at least $1 million CSL • Criminal background checks, no older than one year, for staff that may come in contact with members • Current business license & tax ID or Social Security number

  13. Psychotherapist Using Pets Credentials • Licensed/registered psychologist, behavioral therapist, mental health disorder specialist, social worker, or physical/ occupational therapist • Documentation of training, licenses & certifications

Pet Therapist Credentials • Therapy pets must be: • Examined by a licensed veterinarian within previous 12 months • Certified for therapy purposes • Pets must have at least one year of experience providing pet- or canine-assisted therapy • Current liability insurance certificate with at least $1 million CSL • Adequate space for each animal according to local codes & ordinances

Pet Therapist Credentials Cont’d. • Therapist must have current membership in at least one professional association such as: • American Humane Society or its affiliates • International Association of Canine Professionals (IACP) • Other similar professional association. • Proof of training in pet therapy • Criminal background checks, no older than one year, for staff that may come in contact with members • MTM will conduct background check for volunteers • Current business license & tax ID or Social Security number (if paid for services

Policies & Procedures • Obtaining written consent from a parent, guardian, or legally responsible person before beginning services • HIPAA & the limits of confidentiality in handling all medical & personal information • Formulating & following therapy plan for members, including duration & frequency • Safety training

Policies & Procedures Cont’d. • Fees, financial arrangements & dealing with third-party payers • Discontinuing or completing therapy • Complaint resolution & tracking • Conflict of interest • Dealing with cultural expectations & variations • Assuring quality, measuring outcomes & reporting service quality

Following Credentialing & Contracting • An MTM Care Coordinator will get a referral from member’s health plan for therapy • Care Coordinator will contact therapy provider based on location & specialty • Computer-based program will exchange information regarding member & treatment notes • HCBS-dedicated phone number

  19. Rates • Individual therapy session of at least 45 minutes conducted by a licensed mental health professional or Art Therapist: $75 • Group therapy session of at least 45 minutes conducted by a licensed mental health professional or Art Therapist: $35 per group member • Therapeutic riding session of at least 40 minutes conducted at an equine therapy center by an equine therapist: Public rate of $35-$45
