Definition of a Lab • Laboratory (Lab) – • A separate, distinct area that has been specifically defined as laboratory space by the Supervisor who is responsible and has control of the activities being performed in that space. • Some examples of lab types include: • Hardware • Software • Chemical (Wet) • Power • Testing • Size (large or small) does not matter • Type (hard walled office, cubicle, etc.) does not matter

  3. Who has Responsibilities? • EHSMS Coordinators/Committees • Rollout to facility/organization • Lab Supervisors • Designate Lab Manager • Ensure training is completed • Lab Users • Work safely • Complete Lab User training • Lab Managers • Avaya Lab Safety EH&S Program Implementation Instructions (LAB-WS-PI) • Ensure compliance with Avaya Lab Safety Worldwide Standard (LAB-WS) and other applicable EH&S regulations • Complete Lab Manager training • Designated contact for EH&S issues

Lab-WS Requirements • Complete and maintain the Avaya Lab Survey (LAB-FM-02), which: • Provides Lab contact information • Lists the potential hazards – for emergency response purposes • Includes section for battery inventory • Populates online database accessible to everyone • To access the online Lab Survey interface, see: • http://www.ehssupport.com/ehs_toolkit/labsurvey.aspx (MACTEC Login = Username: avaya2 Password: f4tornado

Lab-WS Requirements • Conduct quarterly lab safety inspections, which includes: • Completing the Avaya Quarterly Lab Safety Inspection Checklist (LAB-FM-01) • Ensuring corrective actions are implemented for items found to be “No” not in compliance • Retaining inspection records for a period of three (3) years

  6. LAB-WS Requirements • Chemical Management • Chemical Inventory (CHM-FM-01) Copy to Facility Management • Material Safety Data Sheet (MSDS) Copy to Facility Management • Proper storage • Proper disposal • Chemical (Wet) Labs (LAB-OG-01)

  7. LAB-WS Requirements • Hand Soldering Workstations • Hand Soldering Operational Guideline (LAB-OG-02) • Signs Required: • No Food or Drink (LAB-CR-03) • Personal protective equipment (PPE) (LAB-CR-02) • Good hygiene practices (LAB-CR-01) • Waste disposal • Regular cleaning
