The HealthQuest Rewards Program offers employees enrolled in the SEHP a chance to earn up to a $480 discount on their 2014 health premiums. To qualify, complete the Health Assessment Questionnaire and earn 20 additional credits from biometric screenings, physician visits, or at-home test kits. New features include the Health Advisor Call and virtual wellness conversations. Participate in wellness challenges and other ongoing programs to complete your credit requirements.   3. Premium Discount – PY2014 • Earning Period: August 1, 2012 – July 31, 2013 • Goal: Health Assessment Questionnaire + 20 additional credits • Incentive: $480 discount on 2014 premium • Who is Eligible: Employees enrolled in the medical portion of the SEHP

  4. 3 Ways to Obtain Biometric Values • Onsite Biometric Screening Events September through June • Physician’s Visit between August 1, 2012 and July 31, 2013 - Print form at http://www.kdheks.gov/hcf/healthquest/download/BiometricsForm.pdf • At-Home Test Kit Ordered Online – allow for mail time and processing

  5. Health Assessment Questionnaire • Required to earn premium discount • Worth 10 Credits - applied to your account immediately • Paper form available by calling help desk at 1-888-275-1205, option 9 – due July 1

  6. What’s New? • Health Advisor Call (5 credits) – only after completeing health assessment questionnaire • Conversations (2 credits/6 max) – virtual conversations completed on wellness portal • Non-Tobacco User Declaration (10 credits) – moved to wellness portal, available all year
