  6. {2014 Activities} • Health Plan Administration Request for Proposal • United HealthCare effective January 1, 2015 • “UMR” technology platform • Focus on data transparency and more informed consumer decision making • Strengthened plan design with more options • 99.15% provider match from Humana to United Healthcare • Members will receive new health plan ID cards late December • Transition Processes • Health Savings Account • Mail Order Pharmacy • Transition of Care 90 day period

  8. {Definitions} • Deductible • Upfront flat amount a member pays before plan begins sharing costs for certain services • Co-insurance • A percentage cost share a member pays for certain services after the deductible is met • Out-of-pocket maximum • The maximum amount paid for in-network deductible and co-insurance covered services in a calendar year • Co-payment • Upfront flat amount a member pays for certain services • Plan Maximum • The maximum amount paid for in-network deductible, co-insurance AND co-payments

  10. {Health Care Reform} • Health Insurance Marketplace (i.e. Exchanges) • Employers required to provide notice to all employees • Ohio has a “federally facilitated exchange” • Metal levels of plans • Platinum, Gold, Silver and Bronze • Criteria for subsidies • Miami’s coverage is minimum value and affordable

  12. {Healthy Miami: Premium Discount Program} • Complete following steps by November 15: • Annual well/preventive physician visit • Health questionnaire • Biometric screening • Age and gender applicable screenings • Tobacco Free Certification • Tobacco free portion of discount ($45) • Completing the university sponsored “Freedom from Smoking” class qualifies for the discount
