  1. One end of a casing joint fell between two sections of pipe rack support, hit a floorman and badly injured his foot. Front section of support had been moved leaving a gap.

  2. Rolling of the casing joint was not done properly. Joint was not rolled square on the pipe rack and so it dropped through the gap

  3. Only part of the support was moved leaving a gap between the two pieces Front part was moved to level pipe rack and make easier to lift onto cat walk

  4. What went wrong? * Joint was not rolled square on pipe rack. * Men did not pay attention to their work. * Set-up of pipe rack not acceptable. Support was wrongly placed. * Supervisor did not monitor work enough.How do we put it right?* Plan the work, work the plan. * Pay attention to the work. * Toolbox Talk - before starting work make sure job is understood by everyone. * Improve supervision.
