Injury Incident: Casing Joint Drop Due to Improper Setup and Supervision
A severe injury occurred when a casing joint fell between two sections of a pipe rack support, badly injuring a floorman's foot. The incident was caused by inadequate rolling of the casing joint, leaving it unbalanced and causing it to drop. The front section of the pipe rack had been improperly moved, creating a dangerous gap. Key failings included lack of attention to work procedures, improper support setup, and insufficient supervision. To prevent future incidents, it's crucial to plan work effectively, ensure proper supervision, and conduct toolbox talks to reinforce understanding of tasks.
