  4. Participants will be able to: • Explain the tasks involved in Phase Five: Implementing and Evaluating the Community Action Plan. • Develop and agree on a final work plan and time line for transitioning to Phase Five. 8-4 Community Planning Training

  6. Secure funding. • Implement tested, effective programs with fidelity. • Complete evaluations. • Plan for Community Board maintenance. • Maintain public relations efforts. 8-6 Community Planning Training

  7. Members of the Funding work group • Members of the Resources Assessment and Evaluation work group • Representatives from agencies involved inplan or program implementation • Outside evaluators • Other Community Board members, as appropriate 8-7 Community Planning Training

  8. Finalize systems-change strategies, and community-level and program-level outcomes. • Complete program investigation and selection. • Address preliminary evaluation plans. • Prepare and distribute the Community Action Plan. 8-8 Community Planning Training

  9. Brainstorm actions. • Assign actions and set deadlines. • For each action, identify necessary resources and any barriers or issues to address. 8-9 Community Planning Training

  10. Include dates for: • key actions from the work plan • the Community Plan Implementation Training • the next Community Board meeting • conferences and community events • other Community Board activities 8-10 Community Planning Training
