Scope of Faith Community Nursing Practice… • “With an intentional focus on spiritual health, the FCN primarily uses the nursing interventions of education, counseling, advocacy, referral, utilizing resources available to the faith community, and training and supervising of volunteers from the faith community.”

  3. Standards of Practice 1 and 2: Assessment and Diagnosis • The faith community nurse… • collects comprehensive data pertinent to the patient’s wholistic health or the situation. • analyzes the wholistic assessment data to determine the diagnoses or issues.

  4. Standards of Practice 3 and 4: Outcomes Identification and Planning • The faith community nurse… • identifies expected outcomes for a plan individualized to the patient or the situation. • develops a plan that prescribes strategies and alternatives to attain expected outcomes.

Summary of the Referral Process Establish relationship with congregant Identify congregant need after assessment (diagnosis) Set clear objectives for a referral (outcomes) Find resource appropriate to goals of older adult Empower older adult/care giver to decide if to pursue referral (planning) Make referral to the resource (implementation) Follow-up to facilitate older adult’s utilization of resource Evaluate referral process and outcome Best Practices
