  2. Keep the “STUDENT COPY” set. • Submit the original set.

  3. ●Fill in the information on P.1. • ● Bring your Student ID Card for verification purpose.

  4. ● Find a witness to sign.

  5. ● Ask your guarantor* to sign on the upper part of P.3. • ● Find a witness* to sign. • *Guarantor and witnessCANNOT be the same person!

  6. ● Provide TWO emergency contacts. Sign on the lower part of P.4.

  7. SETTLE CUHK TUITION FEE BEFORE THE DEADLINE • *A fine of HK$200 will be levied for late payment!

  8. SETTLE ISEP PROGRAMME RELATED-FEES BEFORE THE DEADLINE (For ISEP students only)

  9. Certifying Letter ● A proof of your nominationforthe exchange program. ● Please keep it for record or future use. NOTE: Students who had attended the Briefing Session which was held on 22 April 2014 should already have it.

  10. Student Declaration Form (For Admission Scholarship students only) ● Fill out the details of the authorized person to be contacted for fee settlement if necessary. ● Sign the form.

  11. E-Departure Form ● Complete the form and submitit at least 2 weeks before departure.

  12. Contact Information During Exchange Form E-Contact Information During Exchange Form <To be finalised> ● Complete the form and submitit within 1 month upon arriving at host institution.

  13. E-Verification of Full-Time Enrollment Form ● Complete this part after arriving at your host institution. Submit this form within two months after arrival at host institution or right after course registration, whichever comes first. To download the form: Home(www.oal.cuhk.edu.hk) > CUHK Students> Downloads> Verification of full-time Enrollment form. ● Ask the staff at the host institution to complete this part. ●Email the completed form to OAL.
