  3. What you need in order to participate • Pay to Play $140 ($700 family cap) • Physical Examination Form &OHSAA Authorization Form (4 pages) • Emergency Medical Authorization Form • Athletic Information & Pledge Form • OHSAA Eligibility Checklist • Road Running Release (CC only)

  4. Eligibility Requirements • OHSAA – Must receive passing grades in a minimum of five one credit courses or the equivalent (PE does not count) • FHSD – Must maintain no less than a 2.0 GPA each quarter (A student may use a one time probation if he/she has earned at least a 1.5 GPA)

  5. NEW OHSAA TRANSFER RULE • You must complete an “Affidavit For Bona Fide Legal Change of Residence Form” and be approved by the OHSAA before an athlete is eligible to participate.

  7. Athletic, Extracurricular and Co-Curricular Code of Conduct Participation in these activities is a privilege and not a right

  8. Overview of Athletic, Extracurricular and Co-Curricular Code of Conduct • The policy covers students in grades 7-12. • The slate is not wiped clean when you leave Nagel Middle School.

  9. When is the policy in effect? • The policy applies during the school year 24-7. • The policy applies during summer events (ex. camp) for your sport or activity. • The policy applies during the summer once the date of the official season and/or activity begins.

  10. Self Referral Policy • A student may voluntarily seek assistance with an alcohol, drug, or tobacco addiction by self-referral to their coach/advisor, AD, or school administrator.

  11. Consequences for Violating First Offense: • Removal for 365 days • May apply for reinstatement after two weeks • Under no circumstances may a student be reinstated earlier than two weeks after initial removal.

  12. Consequences for Violating Second Offense: • Removal for 365 days • No possibility of reinstatement until end of 365 days.

  13. Consequences for Violating More than two offenses: • Permanently removed from all activities

  15. Transportation Guidelines • Students participating in athletics are required to ride district transportation to and from contests. • If a student must ride to or from a contest with parents, the student must have a note signed in advance by parents and athletic director.

  19. HERE IS THE DEAL! • Switch to Forest Green @ $49.99/ qtr & $7.50 for recycling • $5/qtr goes to the weight room for 2.5 years ($50/ switch)

  23. The BONUS! • If you can convert 10 neighbors to Forest Green, you will receive FREE pick up for a year. • Think about this for a minute: Reduce expenses and give at the same time without giving out of your pocket!

  24. HOW • Fill out paperwork in Forest Green folder. Start up notice and cancellation notice to other waste company. • You can pay in advance or be billed. • Pick your start date.
