LINKAGE – Working in Partnership to deliver benefits and services • LinkAge is a partnership delivery model for better integration of healthcare, housing, delivery of benefits and social care. The model uses local networks of services and there are three elements: • Joint Teams • Alternative Offices • The Partnership Fund
LINKAGE PLUS PILOT The Government published ‘Opportunity Age – Meeting the challenges of ageing in the 21st century’ in March 2005 ‘Opportunity Age’ recommended the piloting of LinkAge Plus to provide a fully integrated service The pilots would be based on the ‘Sure Start’ model
LINKAGE PLUS CONCEPT • The LinkAge Plus programme will: • Be driven by the needs and aspirations of older people themselves • Expand the principles of joined up working, • Involve a programme of pilots that will integrate services • Build on existing links between central Government, Local Authorities and other organisations
PILOT SELECTION • We plan to run up to 8 pilots in England and have asked a number of local authorities to work with us to develop the LinkAge Plus concept further and advise on how this can be delivered practically on the ground • We are currently working with: • Devon County Council, Gateshead Metropolitan Council, Gloucestershire County Council, Lancaster City Council, Leeds City Council, London Borough of Tower Hamlets, Nottinghamshire County Council, Salford City Council
PROPOSEDINITIATIVES • Range of suggested initiatives Pilots propose to introduce as part of LinkAge Plus Programme • Home Handy person scheme • Mentoring Services • Network Centres (a single integrated accessible gateway to services) • Outreach workers • Village Agents in rural communities • Transport scheme – using taxi voucher with community transport • scheme arranged with Environment and transport providers • Volunteering
LINKAGE PLUS EVALUATION • The LinkAge Plus pilots are expected to run for a period of up to two years. They will: • Test different models of joining up services for older people • Be independently evaluated • The findings will form a body of good practice and lessons learned to encourage wider application beyond pilot sites

LINKAGE PLUS – NEXT STEPS The LinkAge Plus project, together with the first pilot was launched in Tower Hamlets on 3rd July 2006 by James Purnell, the Minister of State for Pensions Reform. The other pilots will rollout over the next few months.
