A Certified MU EHR vs. a Care Management Plan Care Plan • Lays out goals and objectives that the patient/client would like to achieve during treatment. • Used by care management agencies, physical health providers, behavioral health providers, etc. • Allows both the patient/client and medical professional to view progress as well as any problems that arise during treatment. • Currently no standards for sharing information between community, regional, state (SHIN-NY) HIEs EHR • Longitudinal look at information from CDOs where patient has had encounters • Structured Information • Owned by patient or stakeholder • Provides interactive patient access as well as ability for patient to append info. • Standards for sharing information between community, regional, state (SHIN-NY) HIEs (nationwide in future)
