SCHC Details • Medium size FQHC • 27FT Providers/30 PT Specialists • 260 Employees/6 Locations • Primary Care, Pediatrics, Specialty Clinics, Neuropsychiatry, Various Enabling Services • NG EPM January 2004/NG EHR May 2007 • 2 BD Lab interfaces – 1 Dx Document interface • eRx (Surescripts) - K-PACS - Patient Portal • Quality Reporting - Performance Monitoring

  3. Our Approach • Used Homeless Van as beta site • Phased to other departments • 2 week intervals • Productivity hit was significant, allowed for us to apply for and receive scope change for EHR. • About 75% of our providers meet/exceed our clinical goals currently

Effects - Transformation • HIS (Medical Records) • 30 FTE going to 10 • New/Additional duties/responsibilities include: • Annual Clinician Compliance Audits (ICD-9, CPT) • Incomplete medical records tracking. • Updating Order Status upon receipt/Order tracking • Lab Assign – Manually match unassigned labs • Merging of duplicate charts • Scanning and indexing • It’s a whole new ballgame.

  6. What we’re up to now - Scribes • Early in the process • Developing Training process • Finding Nursing school waiting list applicants • Help our Veteran/Technologically Challenged providers • Increased visits/Reduction in Indirect patient care costs. • Quality of life

  7. SharePoint – Data Ownership • Use SharePoint (Microsoft product – Free) • SQL reporting services feeds it. • Helps create a culture of quality • Nurses can see their work for the day • Timely corrections if necessary • Providers can check population management • Transparency - “Motivates the Herd” • Switch – Chip and Dan Heath
