  2. Etiology and pathogenesis 1.food:fish,lobster 2.drug:aspirin,penicillin 3.infections:bacterium,virus 4.physical factors:cold,trauma,sunlight 5.other factors:emotional stress mast cell→histamine

  3. Clinical manifestation 1.acute urticaria:edematous wheal,elevated,whitish or reddish, evanescent(patch or streak),surrounded by a red halo,severe itching,may consist of macular erythema or papules,overlap 2.chronic urticaria:over six weeks

  4. histopathology Epidermis is normal,collagen bundles are separated by edema and there is a perivascular lymphocytic inflammatory infiltrate,mast cell may be increated in number

  5. diagnosis Clinical feature: 1.wheal 2.transitory 3.forming and involuting at the same time 4.severe itching:stinging or pricking sensation

  6. Differential diagnosis 1.papular urticaria:①comman disease of infancy and chlidhood;②fresh lesions are small urticated papules;③persist for several days;④distribution of grouped lesions 2.vasculitic urticaria:①wheal,light itching, ②local pain, ③persist for several days, ④involute→ pigmentating and scale

  7. treatment 1.acute urticaria: ①antihistaminic ②vit-c ③calcium agent ④hormone ⑤external application 2.chronic urticaria:combine antihistaminic
