Pearls of Wisdom Saint Mary’s Case Management / Social Services The Case Management /Social Services team has successfully implemented a readmission tracking, reporting and outcome analysis of high risk readmission patients. CM/SS works closely in a multi-disciplinary approach with physicians, admitting staff, post acute care agencies as well as community providers. CM/SS set a goal to decrease our overall readmission rate to less than 12% which has been successfully demonstrated in the first quarter. The CM/SS team has accomplished this through team accountability and a daily reporting process. We make an attempt to identify these patients before they are readmitted and align them with the appropriate discharge referrals and resources. Using a patient derived approach the team focuses on advocacy, education and self- efficacy.
