STRATEGY OVERVIEW Sustain the Gain • This strategy area is invested in promotion, prevention and early intervention programs for children and youth age 5 to 24 years old. • Adolescent Immunization Learning Collaborative • Preventive Healthcare • Increased Vaccination Coverage
APPROACH Establish a local quality improvement (QI) learning collaborative focused on adolescent immunizations • Partnership between the contracted community based organization and the Public Health – Seattle and King County Immunization Program • Primary aim is to increase adolescent immunization coverage • Core function will be the facilitation of quality improvement intervention cycles with cohorts of participating healthcare practices. • Goal is to create a sustainable model for a durable quality improvement learning collaborative
FUNDING STRATEGY Overview • Average annual investment of $200,000 • One contract to be awarded to one agency for three year period
GENERAL INFORMATION AND REQUIREMENTS
ELIGIBILITY • Experience facilitating education, outreach and promotion of initiatives to healthcare providers • Strong understanding of the health system involved in primary and preventive care for adolescents and teens in King County • Ability to manage ongoing American Board of Pediatrics and/or American Board of Family Medicine Part IV Maintenance of Certification requirements for participating physicians • Knowledge about immunizations recommended for adolescents and teens

INVESTING EARLY • Project Activities/Goals: • Capacity to carry-out identified activities and strategies in order to form a sustainable learning collaborative model • Experience and understanding with regard to gathering, evaluating and applying data in order to facilitate change and drive improvement • Understanding of the issues that contribute to poor rates of adolescent immunization coverage among the general population as well as within subgroups of youth and adolescents, and experience in identifying and applying strategies in order to address them. • Experience in facilitating collaborative partnerships focused on healthcare delivery and community needs • Capacity to achieve the necessary activities within the timeline and funding level proposed • Equity • Application clearly defines disproportionality, disparity/ies, or other gaps in services or approaches focusing on subgroups of youth and adolescents, and how the project will address it.
Using Data • Identify your story. • Decide what information will make your story compelling. • Find the data you need. Be creative! • What data do you have in-house that will help tell your story? • Consider numbers (quantitative data) as well as quotes, stories, or even images and videos (qualitative data).
