Active participation in the Wellness plan and meeting incentives allows employees full Flexible Benefit Dollars to meet the employee’s portion of the premiums.
 
17. Return On Investment Due to employees’ participation, Flanders has been able to maintain the level of health care coverage that our employees have come to value without increases in employee premiums.

In the last 7 years, FE premiums have increased 7%.  That is only 1% a year.  
Premiums for 2007 rose an average of 6.1% nationally
 
18. HOW DO I MEET THE ANNUAL WELLNESS INCENTIVE? Step 1:  Must participate in the annual health risk assessments
If your spouse is or is planning to be on the FE family insurance, the employee and spouse must both participant in the assessment and meet the criteria to receive the financial incentive.  
If your wife is pregnant or 6 months post partum or less, she only is required to complete the “Declination of Tobacco Non-Use” form.   
 
19. HOW DO I MEET THE ANNUAL WELLNESS INCENTIVE? Step 2:  Address any risk factors that were identified after receiving results in the mail
Must contact the Wellness Dept to enroll yourself and/or your spouse in the program
If you do not have any risk factors (0), you automatically qualify for the financial incentive 
20. HOW DO I MEET THE ANNUAL WELLNESS INCENTIVE? Step 3:  Financial incentive is rewarded when the wellness criteria is met before the set deadline.
