Managing Stress and Uncertainty in a Patient with Diabetes and Hypertension

This project introduces Bill Ford, a 56-year-old male with a history of type 2 diabetes and hypertension. He experiences significant stress regarding his health and future, fearing increased risks of heart failure and stroke. The primary health needs identified are psychological support and stress management. Utilizing Neuman Systems Model and Mishel’s Uncertainty in Illness Theory, the goal is to decrease his stress and uncertainty through a treatment plan focused on maintaining positive health behaviors, spiritual self-care, and adaptation.
