  3. MISHAP REPORTING • All mishaps must be reported promptly using the list provided in NYW Supplement to CAPR 62-2. This document shall be available at any activity, in all aircraft and vehicles. • All mishaps must be reported using the online CAPF 78 mishap reporting form which is found on the eService CAP website. The Safety Worksheet should be used to ensure that all information is entered into eServices.

  4. MISHAP REPORTING - continued • The Northeast Region Commander requires notification (within 24 hours) of all mishaps involving aircraft. • Per CAPR 60-1, any member involved in an aircraft mishap, no matter how minor, is immediately grounded until advised by the Wing or Region Commander.

  5. CAPR 62-1 Section 3:Required Safety Program Criteria • Formally appoint a safety officer at each level. • Each Safety Officer will complete the on-line Safety Officer course • Perform a safety survey at least annually – this will be documented using online form in eServices. • Develop a program of regular safety education & accident prevention for units to which they are assigned (15 min/month or 3 hours per year). This will be documented using NYWF-85.

CAPR 62-1 Section 3Required Program Criteria • At least annually, Operational Risk Management (ORM) will be discussed. • Safety briefings will be incorporated in all activities. • All mishaps will be reported in accordance with CAPR 62-2 and NYW Supplement to CAPR 62-2. • An annual safety day will be held in October of each year.

  7. Submittals to Wing • 1. All squadrons must report their safety activities each quarter on NYWF85. Email the completed form to the group safety officer. Note: the attendance records are filed for 2 years – do not send with report . Quarterly deadlines are as follows:Reports  for 1st quarter (Jan-Mar) must be received by the group NLT 5 Apr and forwarded to NYW WG/SE NLT 10 Apr.Reports  for 2nd quarter (Apr-Jun) must be received by the group NLT 5 Jul and forwarded to NYWG/SE NLT 10 Jul.Reports  for 3rd quarter (Jul-Sep) must be received by the group NLT 5 Oct and forwarded to NYWG/SE NLT 10 Oct.Reports for 4th quarter (Oct-Dec) must be received by the group NLT 5 Jan and forwarded to NYWG/SE NLT 10 Jan.

  8. Submittals to Wing • 2. The other required report is an annual safety survey. Each unit must conduct a safety survey during November or December each year. The safety survey shall be submitted online using the form in eServices. • 3. Any mishap (bodily injury, vehicle or aircraft) must be report to the wing director of safety within 24 hours. If there is a question about whether a mishap qualifies as "reportable", please call for clarification – follow procedure in NYWING supplement to CAPR 62-2.

  9. How to submit forms: • Open word documents from the NY Wing Web site (www.nywg.cap.gov – listed under forms in web site). Use NYWF-85 for reporting safety activities. • Fill in data, then save as a word file using the “save as” command – i.e. NY-333 1st Quarter 2008.doc • Remember to complete the boxes in the transmittal section (6) of the form!

How to submit forms: • Forward this word document as “an attachment” to the Group Safety Officer in an email. • The Group Safety Officer will collect all of the reports from the squadron in your group and forward them to me as one email. • Remember handwritten, html, or scanned files (.jpg or .pdf) ARE NOT ACCEPTABLE! Only Word documents, using the proper Wing forms, are accepted by NYWING!

  11. How to submit forms: • If you are having problems with your computer skills – and remember, we are all not experts – try to find a cadet that can help. Many of the younger folks have grown up with computers and are absolute wizards with a keyboard and mouse. We need these forms submitted in this format to effectively collate and transmit to the Wing, Region and National levels. Ask for help!
