  2. 2nd Step in the KEYS Survey • School Registration • Demographics Form • Accepting Surveys • Survey Closes (80% threshold) • Report Generated • Report Made Available

  3. The Demographics Form is: • Completed by School Facilitator. • Required to Launch the Survey. • Sets Participation Target (80% of staff). • Provides data to inform NEA research.

  4. Click Facilitator Login

  5. Enter Email & Password

  6. Click on School Name

  7. Complete Demographics Form

  8. Demographics form – page 1 Enter: * Number of students enrolled * Total number of staff * Number of staff providing direct instruction to students. (number used to calculate response rate) * Average class size. * School level (e.g. elementary, middle, high) * Type of community (e.g. rural, town, suburb, city) * Racial/Ethnic composition in percentages (must total 100%)

  9. Demographics form – page 2 Select Yes or No to indicate whether a standardized test was given in the last 12 months.

  10. Demographics form – page 3 Enter: * name of last standardized test * month/year of last standardized test * month/year results were made available * average score for highest grade level * average score for minority students in highest grade level

  11. Demographics form – page 4 Enter: * average performance level of all students * average performance level of minority students * percentage of students in special education * percentage of students enrolled in ESL programs * percentage of students receiving free/reduced lunch * socio-economic status of most parents * external organizations contacted about school-related matters.

  12. Demographics form – page 5 Select Yes or No to indicate whether you have completed the form and wish to launch the survey
