  3. A national movement to find and house 100,000 vulnerable homeless people by July 2014. • Targets top 50 communities with more than 1,000 persons living outside • Created a new way to prioritize people for housing, and tools match those in greatest need with housing

  4. 237 Communities, 50+ in California • 90,046 housed • as of March 27, 2014

Local Connections • January 7: County Board of Supervisors approved funding to implementation the Vulnerability Index Survey developed by the 100,000 Homes Campaign, to identify the homeless persons living outside who are at greatest health risk. • Coordinated Intake (coming fall 2014): This project allows us to field test a screening tool for Coordinated Intake, as well as a unified housing referral system. • Aligns with federal priorities on chronically homeless and most vulnerable homeless households.

  6. Vulnerability Index Tool • More than 6 months homeless AND at least one of the following: • End Stage Renal Disease • Liver Disease or Cirrhosis • HIV+/AIDS • Over 60 years old • 3 or more emergency room visits in prior three months • 3 or more ER or hospitalizations in prior year • Tri-morbid (mentally ill+ abusing substances+ chronic medical problem) • Additional scoring for families & pregnant women • Generates an acuity score up to 20 to prioritize people by score & generates housing recommendation

Vulnerability Registry Week • Multi-agency & multi-disciplinary team • HIPAA-compliant data collection • Sonoma County Goals • Identify vulnerable individuals and families on the street and in shelters • Create housing placement targets • Improve the way we link vulnerable people to housing in Sonoma County

Volunteer Opportunities • Survey volunteers: 5:00 -8:00 AM (125) • Partner with paid homeless guides • Canvass the 25 census tracts where the densest concentrations of homeless persons were found in 2013 • 15 minute interviews & photo if possible • Data entry volunteers: 8:00 -11:00 AM (65) • Same-day data entry into our Homelesss Management Information System (HMIS) as teams come back. • Sign up • http://sonoma-county-homelesscount.wikispaces.com/Volunteer+Information

  9. 6 Deployment Locations & Schedule Unless noted, sites operate Monday-Wednesday April 7-9 • Family Support Center, 465 A Street, Santa Rosa (goal: 750 surveys) • COTSMary Isaak Center, 900 Hopper Street, Petaluma (goal: 600 surveys); street outreach April 11-13 • Wallace House, 126 N. Main Street, Cloverdale (goal: 100 surveys) • Healdsburg Labor Center, 405 Grove Street, Healdsburg(goal 150 surveys); Data entry at 3850 Montgomery, SR • Empowerment Center, 16390 Main Street, Guerneville (goal 100 surveys) • The Haven, Sonoma (goal 100 surveys) – Date TBD

Data Entry Training • Friday, April 4 In-Person Training Session • 1:30 PM for new EtOUsers • 2:30 PM for all EtOUsers • May bring your laptop to use CDC wireless • We will post on SCORS in the next few days to reserve your place • On-line recording • will be posted at http://sonoma-county-homelesscount.wikispaces.com/Meetings+and+Training+Schedule • EtO leads (“czars”) will be at each site

Data Management & Reports • Efforts to Outcomes client database managed by the Community Development Commission • Highly protected database for client case management and aggregate reporting to homeless services funders • Privacy & security training required • Survey includes release of information including all project partners • Scoring & Data Analysis by CDC staff April 8-14 • Initial Report to Board of Supervisors, Tuesday April 15th

  13. Delivering Housing & Services • This is a “Housing First” program: as much as possible, placements will be made into permanent supportive housing with appropriate services. • Of 1,000 permanent supportive housing beds • ~22 turn over each month • 60% have a preference for chronically homeless & vulnerable persons, if they fit other criteria • Anticipate another 25 beds for chronically homeless & vulnerable by end of 2014, and other new resources • Initial meeting: Friday April 11, 3:30-5:00 PM
