  4. Acute Liaison There is now an Acute Liaison Nurse based at Warwick Hospital This is a joint funded post with the hospital

  5. Acute Liaison Tracey Anderson is the acute liaison nurse for Warwick. Sally Wilson is the Acute Liaison for George Elliot Hospital They help patients who have a learning disability to get the best care

  10. Improve the number of Annual Health Checks GP’s will get paid for completing health checks for people with a learning disability We want your GP to invite you to have a health check every year

  11. How can we help to GP’s to do this? We will complete three training sessions for GP’s and Practice Staff Service users will support us with this training

What the training will cover • What is a learning disability? • How to identify people with a Learning Disability • Understanding the health needs of a person who has a Learning Disability • What a health check should cover • Health action plans • Overcoming barriers faced by people with Learning Disabilities • Mental capacity andDiscrimination Act.

  13. Helping GP Practices • Make sure the Register is correct • Give the GP’s letters to use to invite you to a health check

  14. Helping GP Practices • Pre Health Check Questionnaires will be sent before your health check • A leaflet will be sent explaining what will happen in your health check

  18. Helping Carers • We will offer training to carers about health facilitation • Preparing for medical appointments • Looking out for signs of illness
