Transformation is unprecedented and the need for assistance is increasing. Today HealthCare reform efforts drive changes in medical practices. Physicians have transitioned to EMR’s and offices are “Medical Homes”.

  3. The use of best practices is the core focus of the AHRQ* program. * Agency for Healthcare Research and Quality Practice Facilitator Program Teambuilding and Expertise in Quality Improvement.

About The Program: • Designed and developed by national experts, in coordinationwith AHRQ. • An online program requiring internet and computer access. • Weekly live video seminars and local internships at certified sites. • Professional HealthCare instructor; executive and educational experience. • Distinguished guest speakers HealthCare. ● Frontline Innovation ● Practice Facilitation Program

Role of a Practice Facilitator: • Practice Facilitators (PF’s) develop a strong relationship with medical practices over time, in order to assist them in evaluation and improvement of their overall quality of health care. • PF’s focus on clinical and administrative improvement through performance evaluation and feedback, patient satisfaction surveys, clinician and staff training in quality improvement methods, team building, disseminating best practices, coordination of quality improvement initiatives, and provision of specific materials and resources. June Levine received her Baccalaureate and Master's degree in nursing from Ohio State University.  Her professional career has varied from; executive, managerial, and clinical positions within ambulatory practices, acute care, and community settings. As chief nursing officer, she served as the Director of Clinical Practice for the Community Clinic Association of Los Angeles where she facilitated broad efforts in the area of disease management, quality improvement, and disaster preparedness. At LA Net, a practice based research network, and SUNY Buffalo, NY she has supervised and educated graduating practice facilitators. The Instructor: June Levine, MSN RN

Program Requirements: ● 13 Weeks @1.5 Hours per week ● 40 Hours Field Work Practicum ● 26 Hours Reflective Learning ● Total Hours: 85.5 (8.5 CEU’s) ● Program Fee: $4,000 ●Certified Sites Incentive: $1,000 * Financial Assistance Available ** Must Meet Eligibility Guidelines PROGRAM BEGINS FEBRUARY 11, 2015
