  2. May 24 – May 30 Queretaro May 23, 2014 Operator opens valve to fill internal spray day tank on L1 after cleaning tank. Later, Supv notices a release and informs operator who then closes valve. Releases about 80 liters around area of tank. Found tank float valve had stuck in the open position. Day Tank

  3. May 24 – May 30 Fairfield May 28, 2014 Employee was walking by the ink racks and nearly tripped over conveyor covers that were extending out into the middle of the walkway from under the racks. Able to maintain balance and replaced covers under rack.

  4. May 24 – May 30 Valparaiso May 28, 2014 It was discovered that the air blower mounted to the mezzanine on Mod 1 (near #11 autobagger) had been hit by an ITO. Work order was issued to relocate this blower.

  5. May 24 – May 30 Kent May 29, 2014 Sulfuric acid drum and metering pump at washer had been placed next to an eyewash station. Sulfuric is highly reactive to water and the drum was moved and informed CPO's of importance.

  6. May 24 – May 30 Winston-Salem May 29, 2014 Employee was clearing cans on palletizer 3 and used his arm to clear the cans. His left forearm got stuck between the main mat and the deadplate, pinching his skin and causing a brush burn because the mat was still moving.

  7. May 24 – May 30 Winston-Salem May 29, 2014 Employee was clearing a jam inside of baler # 3, but did not have the unit locked out. Stopped employee immediately and had him perform LOTO before continuing work.

  8. May 24 – May 30 Queretaro May 30, 2014 An operator was trying to remove a can on pallet conveyor and did not notice pallet was moving in her direction. Activity was stopped and we reviewed the risks of the activity and reinforced safety topics with this operator.
