  3. What is a PHR? The PHR is a tool that you can use to collect, track and share past and current information about your health or the health of someone in your care.

  6. Your PHR can make a difference The PHR can enable you to partner with your health care providers, or it can be a private reference used to keep your family informed about your health history.

  7. With your PHR you can: Knowledgably discuss your health with providers Provide information to new caregivers Easily access your health information while traveling Access your information when doctor’s office is closed

  8. With your PHR you can: Record your progress toward specific health-related goals Refer to physician instructions, prescriptions, allergies, medications, insurance claims Track appointments, vaccinations, and other wellness healthcare services

  9. A PHR is not a Medical Record Medical records contain information about your health compiled and maintained by each of your healthcare providers. A PHR is information about your health compiled and maintained by you. Privacy and confidentiality

  10. What is found in a PHR? Diagnosis and problems Medication and any allerigies Medical history Notes about your visit with your doctor Laboratory results Imaging reports Summary of events Family history (especially if anyone else has lupus) Information that you feel is important to your health

  11. Starting your PHR Request copies of your health records Some providers offer a plan for starting a PHR It is not necessaryto gather all information at once Try to gather information from as far back in your treatment as possible
