  3. I. Community Profile Inventory : Location Perspective 1. Boundary of community: - Community location. - Community boundary. - Physical size ( in square kilometers). - Local or folk name. - What smaller communities does it include ?

  4. 2. Location of health service: - Location of health institutions.( Primary health care centers - Hospitals - Rehabilitation centers - Mental health ). - Services of health institutions. - Location and services of major health institution outside the community.

  5. 3. Geographic feature: - Land use ( common areas ; Public and Private space ). - Open spaces ( play area - parks ). - Recreational facilities ( Pre - school ; school aged ; adolescent ; adult ; older adults ).

  6. 4. Climate: - Temperature average and precipitation. - What climatic feature affect health and fitness ?

  7. 5. Plants and Animals : - Plants and animals possess possible threats to health.

  8. 6. Human-made environment : - Types of industry. - Quality of housing. - Air pollution – water pollution – waste ; can influence the environment. - Public sewage – garbage – trash disposal. - Transportation – private ; public transportation.

  9. II. Community Profile Inventory : Population Perspective 1. Size : - Total population number. - Is the community urban ; suburban ; or rural community ?

  10. 2. Density: - What is the density of the population per square kilometers ?

  11. 3. Composition: - Age distribution – sex. - Marital status of the community members. - Occupations are represent and their percentage.

  12. 4. Rate of growth or decline : - How has the population size changed over the past decade ? - What are the health implication of this changed ?

  13. 5. Culture differences : - What is the ethnic breakdown of population? - Do any of sub cultural gropes have unique health needs and practice ?

  14. 6. Social class: - What percentage of population falls into each social class ? - What do class differences suggest for health needs and services ?

  15. 7. Mobility: - How frequently do members move within the community ? - Are there any specific populations ; such as migrant workers ; that are highly mobile ? - How does the pattern of mobility affect the health of community ?

  16. III. Community Profile Inventory : Social System Perspective 1. Health system: - Vital statistics : mortality and morbidity rate ; health problem data. - Health facilities and agencies. - Health related planning groups. - Health manpower.

  17. 2. Educational system : - Public education facilities. - Private education facilities. - Educational services for special populations – adults ; disabled children and adults.

  18. 3. Communication system : - Newspaper ; radio ; TV.
