Manifestation Determination/Behavior Tracking • MDR letter • Required when holding an MDR • Attach safeguards • Attach copy of letter to IEP • Behavior tracking form and instructions • Options: • Create individual student copy and track electronically • Print individual copies and track by hand

Transportation is provided ONLY... • For students who are attending a school other than their home schools due to the nature of the disability and the need for a particular program or service. • For students with a disability so severe that he/she could not safely travel to and from home and school.

  6. Transportation is NOT provided... • For the parents’ convenience. • When general education transportation is available. • For students who have been suspended from general education transportation. • For students attending a school of choice.

Level Change IEPS • Elementary to Middle School • Hold a level change (not transition) IEP • Middle School to High School • 8th grade case manager contacts high school department head • Get information about scheduling/co-taught classes offered • Share list of students and co-taught classes they will need • Hold a meeting with the parent -- this not an IEP • Amend the current IEP to change RR hours • Complete course selection form • Submit amendment and FAPE form to Special Ed office

  8. 2011-12 Elementary Service Model • One TC will be assigned to each elementary school. • No additional staff will be hired. • A realignment of secondary staff will be made to provide the additional staff needed at the elementary level. • The annual assignment request paperwork will be out soon. This form will be used to request a move to an elementary TC position.

  10. IEP Update • There has been a significant improvement in the quality of paperwork.

FAPE • FORM: • Available in Zangle -- Notice of Provision • Fills from the IEP or Amendment: • If the IEP has ‘initial’ checked as the purpose, it will send up an initial FAPE form. • If ‘initial’ not the purpose, the form will be the standard notice of provision -- be careful that ‘initial’ and ‘annual’ are not both checked. • Eligibility -- The appropriate box will be checked on the FAPE. • The amendment check box will fill as required. • Be sure to pay attention to the other options and other factors boxes. • Do NOT check resident/nonresident district boxes if the student resides in the district.

  12. Progress Reports • Progress is reported for an IEP year, not school year. • The IEP date on the progress report should be the date of the PREVIOUS IEP. Progress on those goals are being reported. Please do not change the date. • All progress reports should have the appropriate number of periods. For an IEP that was held a year ago: • Elementary -- 4 periods • Secondary -- 6 periods

  13. FAPE Signatures • Initial • Eligible student • Administrator • Parent -- accept or decline FAPE • Ineligible student • Administrator only • Annual • Administrator only • Amendment • Administrator only • Parents sign the amendment itself. • Check that signatures are dated.
