Training Package • Six two-hour disease specific modules: • Cancer, arthritis, cardiovascular health, asthma, diabetes, and dementia • One four-hour generic module: • Unit one = lifestyle counselling • Unit two = chronic disease self management • Unit three = care planning, cultural sensitivity, info on Medicare item 10997, practice systems
