Transient • Students who attend another institution and plan to enroll at Southwest for one or more semesters and then return to the home institution. These students are not eligible to receive federal financial aid. Official High School Transcript with graduation date • TN grads must pass the Proficiency Exam • Test Scores (ACT, SAT, COMPASS) • ACT and SAT scores are only valid for three years from test date • MMR - Measles, Mumps and Rubella records (Only needed if born after 1956 and full-time) • Proof of Varicella– Chicken Pox (Born after Jan. 1, 1980 and full-time) • Complete Hepatitis B Form • Orientation First Time Freshman
