  2. Background Follow My Health (FMH) from AllScripts has been selected to provide a portal for patients to their UK medical records. FMH Universal Health Record enables the patient to view, download and transmit personal health records. The portal account is the patient’s, not UK HealthCare’s. It lives in the “cloud” and contains what we have chosen to push to it. Therefore, it’s not the patient’s entire medical record with us but rather the parts deemed most understandable, useful to the patient. The patient may have other providers not associated with us pushing data to their portal account.

Background cont. • UK portal initiated as part of the federal Meaningful Use Stage 2 requirements, which dictate: • At least 5% of each provider’s patients (if on EHR) and 5% of our hospital patients should have access to their electronic medical record. • There is a financial cost to UK HealthCare if we do not meet Meaningful Use targets. • Records from both hospitalizationand clinicswill be provided in one portal site for the patient. So far, we are the only organization in Kentucky offering both.

  4. MU2 Requirements During the quarterly reporting period, physicians must: Provide online access to more than 50%of all unique patients. Provide patient-specific education to 10%of patients seen. Ensure more than 5%of patients view, download or transmittheir protected health information (PHI) to a third party. Ensure more than 5%of patients send a secure message.

How the Portal Benefits You • Improved outcomes because patients have access to health education, medication lists, follow up instructions. • Ability to make, cancel or reschedule appointments may decrease the number of “no shows.” • Reduce phone calls/messages. Requests from the portal create tasks in the AEHR managed by the patient care team. • Easy for patient to share correct info with other providers. • Improved patient satisfaction.

  7. Paper vs Electronic: Providers and Patients Information that will be available in the portal from both the perspective of our providers who practice in various environments – paper and electronic – as well as our patients who receive care in various environments – paper and electronic.

Full Launch: June 2, 2014 Remaining clinics and both hospitals. Patient Access to the Portal • Patients will be connected to UK HealthCare’s patient portal once they have: • Accepted their patient invitation and entered their invite code (4 digits of birth year). Invite sent within several days of discharge/visit. • Accepted terms of use, release of info, etc. • Created username and password
