  8. Evidence of improvement maintained over 3 years: (April 2004 – March 2009)

  9. ‘Predictive Modelling’ • Predictive modelling (trend) • health needs modelling eg. Modelling a rise in obesity and model how a rise in diabetes will follow. • Predictive modelling (individuals at high risk) • ie of high risk individuals eg PARR tool and NHS Health Checks; while the focus in on risk stratification by groups, it does then link to individual-level (patient-level) data. • Predictive modelling (activity) • patient pathway activity and design / demand management. • Predictive modelling (impact) • ie predicting the impact of our interventions – eg modelling with Logframes; DH HI tool, this is also where some of the effectiveness work would sit (eg QALYs, etc.).
