  1. How to enroll inCooperative Education

  2. Step one: Complete the application packet • Go to www.lavc.edu/cooped to access the Cooperative Education Application Packet

  3. Student Expectations Contract: • Read the Student • Expectations Contract CAREFULLY. • If you agree to the Terms and Conditions, print your name, sign and date.

  4. Cooperative Education Application: • Complete the Cooperative Education Application. • Enter ALL the information requested.

  5. Time Sheet: • Complete the Time Sheet. • Enter ALL the information requested (including projected work hours).

  6. Grade Sheet: • Enter your name and • Student ID on the • Grade Sheet.

  7. Employer letter & Objectives worksheet • Give the Employer letter and Objectives worksheet to your supervisor (3 pages) • After the letter is read, your supervisor is to complete the worksheet with you, describing what you are going to learn and how you are going to learn it. • Employer initials the bottom portion of the worksheet.

  8. Step two: Make an appointment • Call 818-947-2334 or email coop@lavc.eduto make an appointment to meet with the Cooperative Education Director.

  9. Step three: Meet with the Coop. Ed. Director • Bring your completed application packet to this meeting. • Your application packet will be reviewed by the Director and course objectives will be explained.

  10. Step four: Get your add slip • An Add Slip will be given to you • upon approval of your 3 learning objectives.

  11. Step five: Enroll • Take the add slip to the Admissions Office to register. • After you are enrolled, return the yellow copy of the add slip to the Cooperative Education office.

  12. Additional information • All first time Coop. Ed. students must attend • one Career Advancement Seminar , • one Job Search seminar, and submit • their homework assignments • by due date.

  13. Additional information: • Students taking Coop. Ed. for the 2nd, • 3rd or 4th time will be handed their • assignments at their scheduled • appointment and must be submitted • by the due date.

  14. Grading • Grades are made up of both • employer evaluation of student (55%) and • scores on homework assignments (45%)
