  2. Supportedlivingarrangement • Each person may be able to live according his/her wishes. • He/she should be able to live where and with whom he/she wants and feelswell with him • We support people in the transition from the institution to independentlife

  3. Supportedliving arrangement • Rytmusstarted to provideregistered socialservicein 2010 in Karlovy Vary region as a part ofproject oftransformationofresidential care • today we support 45 people in 7 cities • in 2015 westarted in Prague also • todaywesuppport 15 adults

  4. Ourteams • Isdevidedacording to regions (cities) andaccessability • 8casemanagers/support coordinators(7-8 persons) + 30 assistantsindividuallychosenforevery single person + 2 communitysocialworkers

Characteristicsoftheservice • communitysocialservice • provided to the person athomeorotherplacesaccording his/her needs • oneofthe basic service - law 108/2006 Sb, rules, qualitystandards • fundedfromthegovernment + regional budget, projecteveryyear • paymentfromtheclientsaccordingamountofhourofservice (cofinancing)

  6. 2 targetgroups • Peoplewithdisabilitieswhowanted to leavetheinstitutions • Peoplewithdisabilitieswho do not want to live in theinstitution– preventionofinstitutionalization

Thegoaloftheservice • to enablepeople to live as much independent as possible, take care aboutthehousehold, healthandsafety • to help people to havepossibility to planownlife, beable to askfor support • to enablepeople to live in naturalrelationshipswithfamily , neighbours , partners, friends • To be part oflocalcommunity • To use thelocal public services

Communitywork – part oftheservice • socialinclusion • we created this position two years ago because it was always important but we had never time to do it • wefocus on findingthe role in thecommunityforthe person by definingthegifts and strenghtsofher/him - and matchingthemwithaccesibleplaces in thecommunity

  13. Useful role in thecommunity • cooperationwithotherorganizations (Trashheroes, Brontosaurus movement) • participation in neighborhoodevents • helpourorganization as a AdvisorofRytmus (audit/ evaluationoftheservices, presentationoftheservicesthroughttheirstories) • the use ofvolunteers
