AK GPRA Pilot Project Team History • Operational since 2000 • FY 2004 Alaska submitted GPRA data (women’s health and diabetes) to HIS • FY 2005, 2006, 2007 all national indicators • Focus on data quality • Initially women’s health and diabetes • Expanded focus to entire data generation process for all indicators—including non-GPRA clinical indicators such as medicine education and diabetes prevalence

Region demographics • Bristol Bay region is located in southwest Alaska • Regional boundaries under the Alaska Native Claims Settlement Act (ANCSA) extend about 350 miles north to south, and about 230 miles east to west, and cover about 40,000 square miles (the size of the state of Ohio) • Three of Alaska's major ethnic Native groups - Yup'ik Eskimos, Athabascans, and Aleuts live in the region • 1990 U. S. Census: 6,972 residents • Alaska Native population: 4,639 residents (66%)

Visit Planning • Nurses using iCare for Visit Planning • Create a panel of the patients scheduled to be seen the next day and then quickly view any updates needed for those patients. • Then schedule needed items such as a PAP test, Coumadin level, etc. to be done while they have the patient there.

Case Management • Case Managers use iCare to set up their own panel of patients. • Women’s Health has set up patient lists village by village of women over a certain age • Then filter that list: • Women who have not had mammogram • Women who have not had a recent PAP • Focus on prevention and early detection for older women with increased risk

  12. “Wonderful Wellness Letter” • There is a wonderful wellness letter that can be printed out and given to a patient at the end of a visit (or can be mailed to them) • Gives the patient a summary of current health status and any needed updates such as immunizations, routine cardiac, diabetic, asthma, etc. follow up. • Also includes a list of current medications and future appointments • looking at incorporating it into our Medication reconciliation protocols

  15. Built in 1963 Control of the hospital transferred to Arctic Slope Native Association under 638 contract in 1998 Since the transfer, top scores as a critical facility from JCAHO Critical access facility is only hospital available to residents of an area larger than the state of Washington

  16. Diabetes grant money – brings patients in from surrounding villages Case manager uses iCare to identify care needed While patient is in Barrow (with diabetes travel funds), case manager makes certain they get all exams, immunizations, screenings, etc. iCare in action - Barrow
