1. If the work will involve any non- research (e.g. clinical) activities, an Honorary Contract will be needed; Contact HR at your NHS Trust Will the nature of the work be research only? No Yes If in doubt, ask your supervisor or line manager for advice If in doubt, ask your NHS R & D office for advice CompleteSections 1-3 of your Research Passport,then give the Passport to your supervisor or line manager Your supervisor or line manager completes Section 4 of your Research Passport then returns it to you Are you an employee or a student? Employee Student Submit to Nathan Jones in IoP HR, who will complete Section 5 Submit to Will Fitzmaurice in IoP EST, who will completeSection 5 Have you obtained CRB and Occupational Health Clearance? No Yes HR (for employees) or EST (for students) order CRB and/or OH screening, according to the algorithm of research activity. HR (for employees) or EST (for students) complete pre-engagement checks & return forms to you, with additional evidence of checks, if any Submit Research Passport, & any supporting documentation to your NHS R & D Office February 2011
