Beginning of Treatment Information • Therapist ID/ Staff Code • Date of Intake • Date of First Session • DSM-IV Primary Axis I Diagnosis at Intake • Client’s Age at first session
PEI Outcome Questionnaires • Questionnaires should be filled out at the start of treatment (Pre), and the end of treatment (Post). • Most EBPs will have a General outcome Questionnaire (YOQ, YOQ-SR, OQ) and a questionnaire specific to its Focus of Treatment. • EBPs lasting more than 6 months should also include an update questionnaire every 6 months. • Questionnaire selection is based on Focus of Treatment, EBP and the client’s age at the start of the EBP.
End of Treatment Information • Date of Last Session • Total number of Sessions • Whether the EBP was completed • If EBP was completed, rate level of success • Client’s Disposition • DSM-IV Diagnosis at End of Treatment
Getting started • For practices other than MAP, Triple P, TF-CBT, and IMPACT, data collection starts once you have been trained on the Outcome Questionnaires and your agency has the Outcome Questionnaires in hand. • Data entry of the PEI outcomes information should start once data entry staff have been trained on the PEI Outcome Measures Application.

Building the PEI Outcome Measures Application • Designed a web-based data capture application • No licensing fee to providers • HIPAA Secure data entry • Designed to capture data for a client, in one Focus of Treatment and EBP, by Provider site. • Pulls client information from Integrated System (IS) • Built design based on lessons learned from home grown Outcome Measures Application (OMA) • Different requirements process and development style • System is flexible, well thought out, and has maximum front end validation • Open Episode at the time of data entry not required • No back-end changes to the data, all edits will be done in user interface
Data Entry into PEI Outcome Measures Application • Users log in and select provider number, Focus of Treatment, EBP, and client. • Users will enter Beginning of Treatment information for a client’s treatment cycle in an EBP. • Outcome questionnaires will be presented based on validated ages of the measures and the age of the client at first session. • Data scores collected at start of treatment and end of treatment and every 6 months if an EBP lasts more than 6 months. Responses to individual questions not captured. • Users will enter End of Treatment information including whether EBP was completed, successful, and what happened to the client. • Data for MAP, Triple P, TF-CBT, IMPACT, Nurse Family Partnership (NFP) and non-PEI funded EBPs will not be entered into PEI Outcome Measures Application at this time.
