  2. Llojet e anestezise ne obstetrikendahen ne 2 kategoritemedha: • - analgjezidheanestezi per lindjenormale( vaginale) • Anestezi per lindje me operacioncezarian. Ne metodatjofarmakologjiketeanalgjezisemundtepermendimrelaksimindheushtrimet e frymemarrjes, homepatia, agopunktura, hipnoza, relaksimi ne ujetengrohteetj.

  3. Analgjeziaepidurale • Eshtemetoda me e mire per tesiguruaranalgjeziefektivegjatelindjes.Indikacionikryesoreshtelehtesimiefektiv I dhimbjes.Kadhegjendjetetjeraamtaredhefetalekuperdorimi I kesajanalgjezie do ishteperparesigjateaktivitetittelindjes. Ketojane: lindjet e zgjatura, stimulimi I lindjes,shtatzenite multiple, gjendjetevecantaamtareetj. • Kunderindikacionetperfshijne: refuzimi I pacientes, crregullimet e koagulimit,hipovolemia, stenoza e aortesetj.

  4. Teknika • Dheniaparaprake e likideveintravenozeduhetteaplikohetparafillimitte procedures ne tegjitharastet e perdorimitteanesteziselokoregjionale. Kjo, per tepasurnjevenetegatshme ne rastekomplikacioneshsidhe per teparandaluarhipotensionin.Pozicioni me I pershtatshem per gruandhemonitorimin e RZF eshte ne dekubitus lateral temajte, ndersapozicionindenjurehste me I rehatshem per anestezistin. Shpimibehet ne niveletndermjet L2-L5. Kateteri epidural futetnepermejt ages dhekrijonmundesine e berjes se dozavetefraksionuara per aqkohesaehste e nevojshmeanestezia.Aspirojmengakateteri per tekontrrolluarpozicionindhe per te pare nesenuk del gjakoselikid cerebrospinal. Dozatjane ne varesitedhimbjesdhekohezgjatjes. Hapesiraperiduralepermbanvazagjaku, rrenjet e nervavedhedhjame. Pasiaplikohetdozambajtesenenaduhetteqendroje ne njepozicion lateral sinister.Nderkohe TA duhettematetcdo 5 min per 30 min.

  5. Anesteziadheanalgjeziaspinale • Nje age e holleatraumatikeaplikohet ne nivelet L2-L5. Agejafutet ne linjenmediane ne permjetlekures, inditsubkutan, ligamentitFlavia, hapesiresepiduraledhe ne fund shponduren duke hyre ne hapesirensubaraknoidaleqepermbanlikidin cerebrospinal. Eshteanestezia me e preferuar ne lindjetcezariane.Nekarahasim me ate epidurale ka perparesipasiaplikohet me kollaj, nukndikon ne kontraksionet e uterusit, ka efekteminimale ne hemodinamikedhenuk ka efektemotorike(mpirjetekembeve).Ana negative eshte e kufizuar ne kohe( 2- 5 ore) dhenese do ketenevoje per nderhyrjeoperatoreduhetteribehet.

  6. Komplikacionet e analgjeziseregjionale • Hipotensioni • Dhimbjetpostduraletekokes • Disfunksionivezikal • Dhimbja e shpines

  7. Monitorimikardiotokografikdhevleresimi I RZF • Gjateinterpretimittekujdesshemte RZF mundtezbulojmehipokseminedheacideminefetale. Monitorimidheinterpretimi I frekuenceskardiakefetaleperdoretsinjemjetdiagnostikues. Stetoskopiobstetrikalvleresonfrekuencendheritmin e RZF duke filluarnga java e 27 e shtatzenise. Monitorimikardiotokografikeshtenjeekzaminim I thjeshte, I padhimbshem, qefurnizon me tedhenatevazhdueshme ne kohereale. Ai mundtejete I vazhdueshemoseintermitent. Ai monitoronfrekuencenkardiake e cilaklasifikohet ne bradikarditerende( <100 RZF/MIN), e moderuar(100-110), RZF ne norme(120-160), takikardi e moderuar(161-180), takikardi e rende(>180)
