  3. Assessment • Assessment collecting data about client characteristics know to affect drug therapy Box 3-1 Medication History

  4. Nursing Diagnosis • Deficient Knowledge: drug therapy regimen • Deficient knowledge: safe and effective self-administration • Risk of injury: side effects • Noncompliance: over use and under use • Not concerned with this in pharmacology since there is not a true clinical component

  5. Planning /Goals • Goals should be stated in terms of client behavior • Patient will be able to state why he/she is taking the medication • Patient will be able to state the potential side-effects of medications • Patient will know when to call health care personal

  6. Interventions • Planned activities to help client reach goals • Client will place daily medications in a pill box labeled with days of the week • Medication container will be filled every Sunday night • Client will take pills in the morning before breakfast

  7. Evaluation • Client demonstrated how to put pills into the proper order in the pill box • Client stated he/she has gotten into the habit of placing medications into the pill box on Sunday evenings. • Client states he/she keeps pill box in the bathroom and takes pills in the morning before breakfast

  8. Assignment • Using the medication history form on-line (patterned after Box 4-1 Medication History in your text) • You will do the data collection piece only
